29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Jobsite - On or about February 01, 2018 employees installing metal roofing on 6/12 pitch gable roof without fall protection. The employees were exposed to a fall height of 7-18 feet.

(a) Jobsite - On or about February 01, 2018 employees used a metal extension ladder that was not extended at least 3 feet above the edge of a flat roof. The roof was 10 feet above ground.
