29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a) Jobsite: Interior of building, 2nd level, west wall opening - An employee applying weather proofing material on the upper edges of an unprotected wall opening from the top platform of a Baker scaffold was exposed to fall hazards of approximately 17 feet without the use of a fall protection system.    Noted on or about 2/6/18.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.    a) Jobsite: Wall opening, 2nd floor, west exterior wall - An employee applying Aquaflash to the top, sides, and bottom edges of an unprotected wall opening, measuring 6 feet 1 inch in width, 7 feet 1 inch in height, with an inside bottom edge measuring 35 inches in height, and an outside bottom edge measuring approximately 15 feet above ground level, was exposed to fall hazards without the use of fall protection system.    Noted on or about 2/6/18.
