29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) The shop; on February 7, 2018 and times prior, employees were permitted to perform servicing and maintenance activities on equipment including, but not limited to, the FMB Manuel Titan bandsaw and the FMB Manuel Phoenix bandsaw, when changing saw blades, exposing employees to electrical energy sources and amputation hazards.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.  a)  The shop:  On February 7, 2018 and at times prior, the employer exposed employees to electrical shock hazards in that employees were required to operate equipment such as, but not limited to, FMB Manuel Titan and FMB Manuel Phoenix bandsaws that had power supplied by extension cords used as permanent wiring.  b)  The shop/work table:  On February 7, 2018 and at times prior, the employer exposed employees to electrical shock hazards in that employees were required to work around a work table that had a power strip plugged into an extension cord used for permanent wiring.  D&G Fabrications and Pipe Supports, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 984108, citation number 2, item number 4, and was affirmed as a final order on September 4, 2014, with respect to a workplace located at 8703 Bellingrath Road, Theodore, AL 36582.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met.   a) The Shop:  On February 7, 2018, and times prior, the employer exposed employees to hazardous chemicals and substances that include, but are not limited to, Kleen-Sol.  The employer has not developed and implemented a hazard communication program.  D&G Fabrications and Pipe Supports, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(e)(1), which was contained in OSHA inspection number 984108, citation number 2, item number 5a, and was affirmed as a final order on September 4, 2014, with respect to a workplace located at 8703 Bellingrath Road, Theodore, AL 36582.
