29 CFR 1910.266(f)(3)(viii)(A): The protective canopy shall be constructed to protect the operator from injury due to falling trees, limbs, saplings or branches which might enter the compartment side areas and from snapping winch lines or other objects; Yarding Operations: On February 4, 2018, an employee was working on a Link Belt 3400 loader with the door open while logs were being moved in Unit D at the bottom of the road. A log entered the cab and pinned his right foot on the inside of the cab causing his foot to be crushed.

29 CFR 1910.266(i)(3)(iii): Training content did not consist of the recognition of safety and health hazards associated with the employee's specific work tasks, including the use of measures and work practices to prevent or control those hazards: Adequate training was not provided to employees who conducted the following operations: a) On January 25, 2018, an employee was working at Unit B as a landing chaser pulling cable guide for the main lines. The turn of three logs shifted which caused a log underneath to injure employee who suffered a right broken lower leg. b) On February 4, 2018, an employee was working with a loader-Link Belt 3400 with the door open while logs were being moved in Unit D at the bottom area of the road. A log entered the cab and pinned his right foot on the inside of the cab causing his foot to be crushed.
