29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Warehouse: On or about February 7, 2018, a forklift operator used a slip sheet attachment on the Crown (#2) sit down forklift (s/n 9A188360) to unload skids from trailers without the manufacturer's prior written approval. Certification, with date and method of abatement required. Certification of abatement is required by 29 CFR Part 1903 and is to be submitted within (10) calendar days after the abatement date.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. Warehouse: On or about February 7, 2018, a forklift operator used a slip sheet attachment on the to move skids in the warehouse on the Crown (#2) sit down forklift (s/n 9A188360) and the truck was not marked to identify the attachment. Certification, with date and method of abatement required. Certification of abatement is required by 29 CFR Part 1903 and is to be submitted within (10) calendar days after the abatement date.
