29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident Trucker Bowl Inc. dba Legends Bar and Grill located at 210 North Belt Highway Saint Joseph, Missouri failed to report within 8-hours the death of an employee that occurred on or about February 3, 2018, resulting from a work-related incident. One employee was killed and another was injured in an incident which involved a hospitalization and the incident was not reported to OSHA until February 7, 2018.
