The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level: On or about 14 February 2018, the employer did not ensure that the exit routes were free and unobstructed: a. Exit door #1. Was blocked by metal pipes and a parts washer. b. Exit Door #2. Was blocked by a roll of wire and a wire extruding machine. c. Exit Door #3. Employees were exposed to caught-in hazards likely to result in crushing and/or amputation injury from unexpected energizing, start-up or release of stored energy from various production equipment, such as the deburing machines, nut formers, thread rollers, and power presses. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 14 February 2018, the employer failed to provide adequate machine guarding to protect operator(s) and other employees from caught-in, pinch point, and struck-by hazard(s) while operating or working near the following machines: a. The Hardinge and Cyclematic CTL-27 Lathes - The employer failed to provide adequate machine guarding such as chip or chuck guards to protect employees assigned to operate the two lathes. b. Deburring Machine - No guarding was provided to prevent the operator or other employees from inadvertently contacting the rotating parts of the machine during the operating cycle. c. National Nut Formers, #13 and #14 - The employer did not provide guarding to protect employees against inadvertent contact with the moving parts of the machine. This exposed employees to struck-by, pinch points, and caught-in hazards. d. National Bolt Maker #17 and #19 - The machines did not have guarding to protect employees against inadvertent contact with moving parts. Employees were exposed to in-running pinch points where the spool of material is drawn into the machine during the operating cycle. e. Multi Head Drill Press - The machine did not have a guard to protect employees from inadvertent exposure to the point of operation during operating cycles while hand feeding and operating the machine with a foot trip switch. The employees were directed to place part into the machine and press the foot pedal, which activates pneumatic jaws that hold the part in place while the drill press lowers. f. Waterbury Thread Roller #9 - The employer did not guard the machine to prevent employees from inadvertently contacting moving parts during the operating cycle. Abatement documentation is required in accordance with the 29 CFR 1903.19(d).

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. On or about 14 February 2018, the employer failed to ensure that the tongue guards were properly adjusted on two (2) Baldor pedestal grinders. Both tongue guards were set in excess of one-fourth (1/4) of an inch from the rotating grinding wheels. This exposed employees to a struck-by hazard in the event of wheel breakage. See Table O-10: On or about 14 February 2018, the employer did not provide and insure the use of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on all mechanical power press(es), including Press #43. The presses did not have guarding to protect the employee(s) against inadvertently having their hands in the pinch point created during operating cycles. The machine was equipped with only one pullback for the use by the operator. This allowed the operator's free hand to be in the die space, exposed amputation. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: On or about 02 February 2018, the employer failed to guard the pinch points created at the point of operation of three Flycutter "Drill Presses" #21, #22, and #23. These machines were operated by a footswitch that left the employees' hands exposed. The employees are directed to place parts into the machine and press the foot pedal, which activated pneumatic jaws that held the part in place while the drill press lowered and completed the operating cycle. This exposed employees to caught-in, pinch points, and struck-by hazards. Abatement documentation is required in accordance with the 29 CFR 1903.19(d).
