29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse.    Storage rack system at the main warehouse: Employees were exposed to struck-by hazards from materials.    The following materials were not adequately stacked, tiered and/or limited in height to prevent sliding and collapsing:    a) Aisle 18: a wooden pallet that held boxes of medical supplies that was stacked on the top shelf had a box sticking out creating an unstable load.      b) Aisle 22: a wooden pallet that held boxes of medical supplies that was stacked on the second tier of the shelf had damaged boxes creating an unstable load; a second stack of boxes of medical supplies that was stacked on the second tier of the shelf had a box sticking out and other boxes scattered on top of the wooden pallet creating an unstable load.      c) Aisle 23: two wooden pallets that held boxes of medical supplies that were stacked on different shelves were not properly interlocked, had boxes sticking out and scattered creating an unstable load.      d) Aisle 25: a wooden pallet that held boxes of medical supplies that was stacked on the third tier of the shelf had one stack of boxes sticking out creating an unstable load.      e) Aisle 26: a wooden pallet that held boxes of medical supplies that was stacked on the top tier of the shelf had a stack of boxes, the boxes two boxes were sticking out and some boxes were scattered creating an unstable load; a second stack of boxes of medical supplies that was stacked on the second tier of the shelf was not interlocked, had damaged boxes and two boxes sticking out creating an unstable load; a third stack of boxes of medical supplies that was stacked on the fourth tier of the shelf had one stack of boxes that was not interlocked and had one box sticking out creating an unstable load.      f) Aisle 27: a wooden pallet that held boxes of medical supplies that was stacked on the fourth tier of the shelf had one stack of boxes that was not interlocked, had two displaced boxes sticking out creating an unstable load; an additional two stacks of boxes of medical supplies that were stacked on the third and fourth tier of the shelf were not interlocked and had boxes sticking out creating an unstable load; a wooden pallet that held boxes of medical supplies that was stacked on the third tier of the shelf was sticking out about 1 foot from the shelve creating an unstable load.      g) Aisle 28: a wooden pallet that held boxes of medical supplies that was stacked on the third tier of the shelf had one stack of boxes that was not properly interlocked, had one displaced box sticking out creating an unstable load; an additional stack of boxes of medical supplies that were stacked on the fifth tier of the shelf were not interlocked and had a box sticking out creating an unstable load; a box sitting on the fourth tier was sticking out from the shelf.      h) Aisle 29: a wooden pallet that held boxes of medical supplies that was stacked on the third tier of the shelf was not properly interlocked and had boxes sticking out creating an unstable load; an additional stack of boxes of medical supplies that were stacked on the fifth tier of the shelf were not balanced, properly interlocked and had a box sticking out creating an unstable load; a wooden pallet that held boxes of medical supplies that was stacked on the fourth tier of the shelf was not properly interlocked and had four boxes sticking out creating an unstable load.      i) Aisle 30: a wooden pallet that held boxes of medical supplies that was stacked on the third tier of the shelf was not properly interlocked and had a box sticking out creating an unstable load.      j) Aisle 31: a wooden pallet that held boxes of medical supplies that was stacked on the second tier of the shelf had all boxes scattered and not interlocked; an additional stack of boxes of medical supplies that were stacked on the second tier of the shelf was not interlocked, had damaged boxes on top of the stack and had open spaces right in the middle of the stack creating an unstable load.        k) Aisle 32: a wooden pallet that held boxes of medical supplies that was stacked on the third tier of the shelf was not properly interlocked and had boxes sticking out creating an unstable load; a second stack of boxes of medical supplies that was stacked on the fourth tier of the shelf was not properly interlocked and had a box sticking out creating an unstable load.      l) Aisle 33: a box sitting on the fourth tier was not interlocked and sticking out about 6 inches from the shelf; a second stack of boxes of medical supplies that was stacked on the fifth tier of the shelf was not properly interlocked and had two boxes sticking out creating an unstable load.      m) Aisle 34: a wooden pallet that held boxes of medical supplies that was stacked on the second tier of the shelf had damaged boxes that collapsed in the shelf creating an unstable load; a second stack of boxes of medical supplies that was stacked on the second tier of the shelf was not interlocked and boxes were scattered on top of the wooden pallet creating an unstable load; a third stack of boxes of medical supplies that was stacked on the fifth tier of the shelf was not interlocked and boxes were sticking out creating an unstable load.      n) Aisle 35: a wooden pallet that held boxes of medical supplies that was stacked on the fifth tier of the shelf were not interlocked and some boxes were sticking out creating an unstable load; a second stack of boxes of medical supplies that was stacked on the fifth tier of the shelf was not interlocked and boxes were sticking out creating an unstable load.      o) Aisle 38: a wooden pallet that held boxes of medical supplies that was stacked on the fourth tier of the shelf was not properly interlocked and boxes were sticking out creating an unstable load.
