29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  a) Side of Building - On or about February 7, 2018, employees installing gable on the building used the midrail of an elevated personnel basket as a scaffold platform in that they worked while standing on the midrail.  The midrail was less than 18 inches wide, thus exposing employees to fall hazards of up to 13 feet onto dirt ground.

29 CFR 1926.451(e)(8): Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface:  a) Roof, Side of Building - On or about February 7, 2018 - Employees accessed the roof of the building by using the top guardrail of a personnel platform attached to a CAT Telehandler forklift and the roof was more than 24 inches above the top guardrail, thus exposing the employee to falls of up to about 14 feet onto dirt ground.
