Lift Works, Inc.: The employer did not report an in-patient hospitalization, that was the result of a work-related incident to OSHA within twenty-four (24) hours for an injury that occurred on January 29, 2018. An employee suffered severe injuries on January 29, 2018 resulting in in-patient hospitalization, when the powered industrial platform, Nar Jib Boom, model #LWI2745, being unloaded from the truck bed, tipped over and fell on the employee.

Lift Works, Inc.: On February 8, 2018, the employer failed to record an injury within 7 calendar days of receiving information that a recordable injury or illness had occurred. A recordable injury occurred on January 29, 2018, when the powered industrial platform, Nar Jib Boom, model #LWI2745, being unloaded from the truck bed, tipped over and fell on the employee causing severe injuries.
