29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  (Construction Reference 1926.103)  a) At the jobsite, on or about and at times prior to February 6, 2018, the employer did not provide basic advisory information contained in Appendix D of the Respiratory Protection standard to an employee who used a filtering face piece respirator (dust mask) on a voluntary basis, exposing the employee to respiratory and dermal hazards.
