29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  (a) Inside house framed wall on west side - On or about February 8, 2018 employees working from the first floor roof rafters 9 feet high used framed wooden wall bracing as means to gain access and descend from the roof.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) Front and east side of house - On or before February 8, 2018 employees framing the steep roof were working 14 feet above the ground with no fall protection.  Juan Gonzales was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1249034, citation number 1, item number 1 and was affirmed as a final order on 9/18/2017, with respect to a workplace located at 7520 Sweet Wine Ct. Gulfport, MS. 39503.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) East side of house - On or about February 8, 2018 employees framing the roof gained access using the ladder that was leaning against the side of the house wall that did not extend 3 feet past the edge of the roof.  Juan Gonzales was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1249034, citation number 1, item number 2 and was affirmed as a final order on 9/18/2017, with respect to a workplace located at 7520 Sweet Wine Ct. Gulfport, MS. 39503.
