29 CFR 1926.501(b)(4)(i): Each employee on a working/walking surface was not protected from falling through holes(including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: On or about February 7, 2018, and at times prior thereto, an employee was working on a rooftop next to a skylight and was not protected from falling over 6 feet through the skylight hole.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about February 2, 2018, three employee working on the rooftop of the commercial property under renovation had not received training on the use of fall protection prior to being allowed to work in elevated places, exposing the employee to fall hazard of approximately 22 feet.
