OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from potential struck by and/or crushing hazards from falling loads and/or lift parts: (a) Facility, 3265 Babcock Boulevard, Pittsburgh, PA: On or about February 8, 2018 the employer did not ensure that the operator(s) ceased operations of the Ammco drive on 12000 lb. capacity automotive lift, Model LMT-12, Serial No. P00000720, when it had been determined that there were broken components in the locking devices. There was a broken rod in the drive system for the locking devices and one lock did not function properly. The employees used a screw driver to jimmy the lock to secure or release it during use. The lift had not been taken out of service until repairs could be made.

29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition: (a) Facility, 3265 Babcock Boulevard, Pittsburgh, PA: On or about February 8, 2018 an airline set to 125 psi was used to supply pneumatic tools and had bubbles and holes and the air was leaking thereby not being kept in a safe condition.
