OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from potential struck by and/or crushing hazards from falling loads and/or lift parts:  (a) Facility, 3265 Babcock Boulevard, Pittsburgh, PA:  On or about February 8, 2018 the employer did not ensure that the operator(s) ceased operations of the Ammco drive on 12000 lb. capacity automotive lift, Model LMT-12, Serial No. P00000720, when it had been determined that there were broken components in the locking devices.  There was a broken rod in the drive system for the locking devices and one lock did not function properly.  The employees used a screw driver to jimmy the lock to secure or release it during use.  The lift had not been taken out of service until repairs could be made.
