29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.      On or about February 8, 2018, the employer did not ensure energy control procedures were utilized when machine operators performed service and maintenance (set up) operations on the 35 Hooper Printer Slotter Machine and the S&S Partition Slotter Machine. 29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:   On or about February 8, 2018, the employer did not ensure machine operators performing service and maintenance (set up) operations on the 35 Hooper Printer Slotter Machine and the S&S Partition Slotter Machine implemented energy control procedures by; isolating the machine from the energy source(s) [per 1910.147(d)(3) requirements], affixing a lockout device(s) [(d)(4)(i)], and verifying de-energization and isolation had been accomplished prior to setting up the machines [(d)(6)].  Energy sources include, but not limited to, electric power.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.     On or about February 8, 2018, the employer did not conduct periodic inspections of the energy control procedures to ensure machine operators performing service and maintenance (set up) operations on the 35 Hooper Printer Slotter Machine and the S&S Partition Slotter Machine were following the procedures and the requirements of the standard for energy isolation and control.  Energy sources include, but not limited to, electric power.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.     a.  On or about February 8, 2018, the employer did not ensure one or more methods of machine guarding was adequately provided to protect machine operators from hazards created by ingoing nip points and/or rotating parts of the feeder portion of the 35 Hooper Printer Slotter Machine.        b.  On or about February 8, 2018, the employer did not ensure one or more methods of machine guarding was adequately provided to protect machine operators from the hazards created by the point of operation of the rotating slotter discs of the S&S Partition Slotter Machine.      c.  On or about February 8, 2018, the employer did not ensure one or more methods of machine guarding was provided to protect packer employees from the ingoing nip points created by the S&S Partition Slotter Machine conveyor belts as they cycle around the end pulley.      d.  On or about February 8, 2018, the employer did not ensure one or more methods of machine guarding was provided to protect machine operators from the point of operation ingoing nip point created the feed roller(s) of the of the Universal Gluer Machine.
