29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  (Construction Reference 1926.103)  (a) Employees who voluntarily wore N95 disposable filtering facepiece respirators while mixing mortar were not provided with the basic advisory information on respirators contained in Appendix D of the respirator standard.  ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(2) and (h)(3) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a)  Employees who were exposed to mortar had not been provided with the following information and training on hazardous chemicals:  INFORMATION  * The requirements of the hazard communication standard * Location and availability of the employer's written hazard communication program * Location and availability of safety data sheets  TRAINING  * Details of the employer's written hazard communication program  (b) As of 02/07/18, employees had not been trained on the new chemical container label elements and the new format and types of information on each section of safety data sheets (SDSs) with respect to revisions to the standard in accordance with the Globally Harmonized System of Classification and Labeling of Chemicals (GHS).  ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1926.51(f)(1): The employer did not provide adequate washing facilities for employees engaged in the application of paints, coatings, herbicides, or insecticides, or in other operations where contaminants may be harmful to the employees.  Such facilities were not in near proximity to the worksite and was not equipped as to enable employees to remove such substances:  (a)  Adequate washing facilities were not provided for employees who were engaged in mixing, transporting, and applying mortar during patch work at the job site.  Potable water was available through water hoses used at the site, but soap and hand towels were not provided. This condition would hinder employees in removing mortar from exposed skin surfaces.
