29 CFR 1926.1053(b)(7): Ladders were used on slippery surfaces and were not secured or provided with slip-resistant feet to prevent accidental displacement. Slip-resistant feet shall not be used as a substitute for care in placing, lashing, or holding a ladder that is used upon slippery surfaces including, but not limited to, flat metal or concrete surfaces that are constructed so they cannot be prevented from becoming slippery: On or about 2/1/2018; Location: Inside Prologis building, 1) Employee fireproofing ceiling pipes at a height of approximately 15 ft was using the fly section of an extension ladder to reach the pipes. The fly section of the extension ladder had plastic ends and was not provided with non skid feet. This ladder was not adequate for concrete floor surfaces.

The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards: On or about 2/1/2018; Location: Inside Prologis building, 1) Employee fireproofing ceiling pipes at a height of approximately 15 ft was using the fly section of an extension ladder to reach the pipes. The employee was not trained properly and was unable to recognize the hazards related to improper use of ladders.
