29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: (a) On or about February 5, 2018, Wood Partners LLC, as the controlling employer, did not ensure that employees of AGL Construction Company, subcontractor, were protected from shock and other electrical hazards from the extension cord that was connected to a generator and powered the DeWalt masonry saw. The extension cord was laid across the path of construction traffic, and was run over by vehicles including the Superintendent's personnel cart (gator) and a fully loaded wheel loader. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
