29 CFR 1926.200(c)(1): Caution signs or other physical warnings, such as caution tape were not effectively used to warn against potential hazards or to caution against unsafe practices, such as working in too close of proximity to an initial line break of a piping section that previously contained a hazardous chemical: (a) Sulfur Dichloride Tank Area - On or about February 8, 2019, the Employer did not utilize sufficient signage, physical warnings, or other methods to ensure that all employees remained a sufficient distance away from the area of a first line break where corrosive chemical exposure could occur.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a) Sulfur Dichloride Tank Area: On or about February 8, 2018 the employer did not ensure that all employees used appropriate eye and face protection during the demolition of a process line that had previously contained corrosive sulfur dichloride liquid in that maintenance mechanics standing near the area of a first line break were only wearing safety glasses as their form of eye and face protection.
