29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. a) On or about February 9, 2018 and at times prior thereto; employees received hands-on forklift operator training but did not participate in training that combined classroom, hands-on and knowledge testing. Allegiant Powder Coating, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(2)(ii), which was contained in OSHA inspection number 1057575, citation number 2, item number 2a and was affirmed as a final order on (9/11/2015), with respect to a workplace located at 1113 N 39th St, Nampa, ID 83651.
