29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   a. On or about February 12, 2018, at the manufacturing area of the above addressed worksite, employees were exposed to a caught by hazard while working with a drill press (i.e. Jet, Model No. JDP-17MF) that lacked point of operation guarding.   b. On or about February 12, 2018, at the manufacturing area of the above addressed worksite, employees were exposed to a caught by hazard while working with a drill press (i.e. Powermatic, Model No. 2000, Serial No. 020900019) that lacked point of operation guarding.   c.  On or about February 12, 2018, at the manufacturing area of the above addressed worksite, employees were exposed to a caught by hazard while working with a roll forming machine (i.e. aluminum panels, 24 gauge) that lacked a point of operation guard on its cutting area.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap:   On or about February 12, 2018, at the manufacturing area of the above addressed worksite, an electrical cord for a pedal of a grommet machine  (i.e. Edward Segal Inc. Model No. 926) had a splice covered with electrical tape, exposing an employee to an electrical fire hazard.
