29 CFR 1926.404(a)(2): A grounded conductor was attached to a terminal or lead so as to reverse designated polarity: a) VA Medical Center Richmond, Intensive Care Services New Addition: On or about 02/12/2018, an emergency power duplex receptacle used by subcontractors was wired with the hot and neutral wires reversed thereby exposing employees to a potential shock hazard. No abatement certification or documentation required.
