29 CFR 1910.30(a)(3)(iv): (iv) The employer did not train the employees in the correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer: Instances including, but not limited to, the following: a) On or about February 13, 2018, during window washing activities, the employer did not ensure that employees were using fall protection equipment correctly in that an employee was tied off to a D ring on the front of his harness for use in a fall arrest system where the harness manufacturer did not design the system to be used in this manner. b) On or about February 13, 2018, during window washing activities, the employer did not ensure that employees were using fall protection equipment correctly in that an employee used multiple connective devices as part of his fall arrest system, one of which was a carabiner whose gate screw securement was damaged, leaving the gate vulnerable to a single action release.
