29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) Jobsite - The employer did not ensure that all employees working on the upper level of a garage were protected from falling through a floor hole exposing the employees to a fall of approximately eleven (11) feet. Observed on or about 19 OCT 2017.
