29 CFR 1918.32(b): The edge of a hatch section, or of stowed cargo, was not guarded by a vertical safety net or other means providing equal fall protection to exposed employees.  (a)  Offload hatch:  On or about February 7, 2018 and times prior thereto, a two foot section of the hatch was not guarded when employees worked in the area.  This exposed employee to a 10 foot fall to the freezer section.

29 CFR 1918.62(g)(2)(iii): Synthetic web slings were not removed from service when they exhibited snags, punctures, tears, or cuts.  Freezer section:  On or about February 7, 2018 and times prior thereto, one leg on a brailer bag had tear half way through the synthetic sling.  This exposed employees to struck-by hazard it the sling broke while off loading fish product.
