29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Production Area - On or about February 7, 2018, employees were required to operate the Liqui-Box C1000 Series Filler Machines on the C Line. The employer did not provide guarding to protect employees from coming in contact with the moving parts on these machines. Employees were thereby exposed to struck-by, caught-in and amputation hazards.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed:  a) Production Area - On or about February 13, 2018, authorized employees were required to perform maintenance and/or service on various equipment, such as Liqui-Box C1000 Series Filler Machines. The employer did not certify that periodic inspections of the energy control procedures were performed at least annually as required by the standard.
