OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to caught-by hazards: On or about February 7, 2018, employees performing service and maintenance activities on gin stands were not required by the employer to apply locks to isolate hazardous energy sources. This exposed employees to amputation hazards.

29 CFR 1928.57(a)(6)(iii): Instruction was not provided to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment. On or about February 7, 2018, gin stand #4, south gin, employees engaged in cleaning were exposed to caught-in hazards because the gin stand was not locked-out/tagged-out from the machine.
