At the Mosaic ammonia facility, an evaluation of the contracted company's safety performance and program was not conducted. A subcontractor was hired to remove and repair the 8 150# Orbit Valve (MOV P2A) on the ammonia system causing an ammonia release incident, on or about December 14, 2017.

At the Mosaic facility located on, 4873 Port Sutton Rd., Tampa, FL, an inspection had not been conducted to the 8" 150# Orbit Valve (MOV P2A), to determine that the valve's, inner components, had been incorrectly installed during prior maintenance. On 12/14/2017 an ammonia release incident occurred in an attempt to repair the valve.
