29 CFR 1910.136(a): The employer did not ensure that each affected employee used protective footwear when working in areas where there was a danger of foot injuries due to falling or rolling objects, or objects piercing the sole. On or about January 25, 2018, employees were exposed to amputation hazards when the employer did not ensure employees use steel toed footwear while utilizing overhead cranes.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees were exposed to struck-by hazards from rotating parts while operating vertical lathes in the plant in the following instances: 1) 50 inch Bullard vertical turret lathe. 2) 74 inch Bullard vertical turret lathe.
