Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. (a) on the jobsite - employees performing roofing work on a 4 in 12 sloped roof of a 2-story apartment building were exposed to an 18 ft fall hazard, in that, fall protection was not installed and utilized; observed on or about 2/8/18.
