29 CFR 1926.605(b)(2): Employee(s) was/were not provided safe access (i.e., ramp or walkway) to or from the wharf, float, barge, or river towboat: Barge "Swiftwater", UMC Dock Extension: On or about February 6, 2018 and at times prior thereto, safe access was not provided for employees that transited from the barge "Swiftwater" to the dock extension. This condition exposed employees to drowning hazards.

29 CFR 1926.605(d)(3): Employees walking or working on the unguarded decks of barges were not protected with U.S. Coast Guard-approved work vests or buoyant vests: Barge "Swiftwater", UMC Dock Extension: On or about February 6, 2018 and at times prior thereto, employees were observed wearing unsecured buoyant vests while transiting from the barge to the dock work area. This condition exposed employees to drowning hazards.
