29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. At the jobsite located at 16600 S Theden St Olathe KS, employees engaged in the cover removal process for sheet metal installation for intake ductwork were exposed to fall hazards of approx. 42 feet in that a convention fall protection systems were not installed, implemented, or utilized. The opening was approx. 108" X 66".

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. At the jobsite located at 16600 S Theden St Olathe KS, On or about 9 Feb 2018, an employee engaged in sheet metal installation stepped into a hole measuring approx. 108" X 66" causing him to fall approx. 26' to a steel insulated panel and another 16' to the concrete below.
