29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about February 14 2018, an employee was operating a Genie GTH-1056 all terrain forklift without wearing a hard hat exposing employees to the hazard of being struck by vehicle roll over protective systems and or roofing materials. Roofers who were not wearing head protection were potentially exposed to falling tools and materials while accessing the roof.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about February 15, 2018, and times prior thereto, while performing roof tile removal work on a roof with a pitch of approximately 10 to 12 and approximately 50 feet above the ground, employees utilized no personal fall protection or fall arrest system exposing employees to a fall hazard.

29 CFR 1926.602(c)(1)(vi): All industrial trucks in use shall meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.  a) On or about February 14, 2018, Genie GTH 1056 all terrain forklift with unapproved job-made trash box with no load rating used by workers loading it with ceramic tiles creating an unstable and off-center load exposing employees to the hazard of being struck by falling debris. Section 605(a) of American National Standard Institute, B56.1-1969 Safety Standards for Powered Industrial Trucks: Handle only stable or safely arranged loads. When handling off-center loads which cannot be centered, operate with caution.   b) On or about February 14, 2018, the operator was exiting the controls to check positioning and left the Genie GTH-1056 all terrain forklift unattended with the boom fully extended to the roof line as workers put pieces of removed ceramic roof tiles in the trash box exposing employees to the hazard of being struck by falling debris.
