29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) New residential construction located on Lot #14, Top plate front side: On or about 02/14/2018, and at times prior, the employer exposed employees to fall hazards of 21-feet, 3-inches to the lower level, in that a complete fall protection system was not used while engaged in roof truss installation.     (b) New residential construction located on Lot #14, Top plate rear side: On or about 02/14/2018, and at times prior, the employer exposed employees to fall hazards of 30-feet, 10-inches to the lower level, in that a fall protection system was not provided while engaged in roof truss installation.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) New residential construction located on Lot #14, First floor access to second level: On or about 02/14/2018, and at times prior, the employer exposed employees to fall hazards of up to 11-feet, 5-inches, in that the ladder used to access the upper level was not extended at least three feet or secured to the structure to allow access to both floors. The ladder used was extended 1-1/2 feet above the second level landing.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employee:  (a) New residential construction located on Lot #14, Temporary electrical service box on front side: On or about 02/14/2018, and at times prior, the employer failed to inspect all of the electrical cords prior to the start of the work shift. Two 50-ft. extension cords were used to power circular saws and other equipment. Both cords used had missing ground pins which had not been removed from service or repaired.
