29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Royal Coffee New York, Inc. 661 Hadley Road, South Plainfield, NJ site - Warehouse: The employer failed to obtain the forklift manufacturer's written approval prior to using shop made man-lift attachments on the Toyota PIT used to assist with stacking bags of coffee. Condition noted on or about 2/12/2018.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) Royal Coffee New York, Inc. 661 Hadley Road, South Plainfield, NJ site - Warehouse: The employer did not ensure forklifts with man-lift attachments marked to identify the type of attachment and showed the approximate weight of the forklift and the attachment combination at maximum elevation with load laterally center. Forklift identification plates did not reflect the load limits of the equipment. Violation observed on or about 2/12/2018.
