29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Employees engaged in maintenance, setup, mold changing, and mold building activities at the following equipment but not limited to the Engel Injection Molding machine, the Toyo Injection Molding machine, the Cincinnati Plastic Injection Molding machine, and the Reed Injection Molding machine in the Production area, the Maintenance Shop, and the Tooling Shop area were exposed to electrical, struck-by and caught-between hazards in that the current machine specific energy control procedures were inadequate and incomplete in such a way that all energy sources (hydraulic, gas, chemical, water, pneumatics) were not clearly addressed; and the lockout attachment points for each energy source were not identified.    29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.
