29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection  a) On or about February 14, 2018; throughout the site; employees were operating portable equipment such as but not limited to Makita 5007F circular saws that were not protected by a ground-fault circuit interrupter exposing them to a shock hazard.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about February 14, 2018; roof area of new addition; employees were not wearing fall protection while installing the rafters and conducting demolition of the residential roof exposing them a 10-30 foot fall                         b) On or about February 14, 2018; first floor; employees were working near a floor opening that was not guarded exposing them to a 10-foot fall to the lower level.   No abatement certification or documentation required for this item.
