29 CFR 1910.134(c)(2)(i): The employer did not provide respirator users with information contained in Appendix D to this section when the employer determined that any voluntary respirator use was permissible:  (a) Employees engaged in foam application activities were supplied with filtering face pieces for voluntary use were exposed to health hazards in that  Appendix D information was not provided.  (b) Employee engaged in plasma cutting and painting activities supplied with 3M 6200 tight-fitted half mask respirator for voluntary use was exposed to health hazards in that  Appendix D information was not provided.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  (a) Employee engaged in alloy plasma cutting and painting activities voluntarily applying 3M 6200 tight-fitted half-mask respirator was exposed to health hazards in that there was no measurement to ensure employee was medically able to apply such protection.  (b) Employees engaged in alloy plasma cutting and painting activities voluntarily applying 3M 6200 tight-fitted half-mask respirator were exposed to health hazards in that there was no respiratory protection program established.

29 CFR 1910.1027(k)(1): All surfaces shall be maintained as free as practicable of accumulations of cadmium:  (a) Employees using Coca-Cola vending machine located in the break room were exposed to health hazards in that the surface of vending machine slot accumulated 0.8540 microgram per 100 cm2 of cadmium.  (b) Employees using Premier Food vending machine located in the break room were exposed to health hazards in that the surface of vending machine slot accumulated 0.2820 microgram per 100 cm2 of cadmium.

29 CFR 1904.41(a)(1): Annual electronic submission of part 1904 records by establishments with 250 or more employees. The establishment had 250 or more employees at some time during the previous calendar year, and this part required the establishment to keep records, but the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form:  On or about 2/15/2018 the employer failed during calendar year 2016, to electronically submit information from their OSHA Form 300A or equivalent by December 15, 2017.  a). The establishment employed greater than 250 employees during calendar year 2016.
