29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two:   a. Site - A horizontal lifeline system which consisted of a lifeline, designed to be used vertically, that was connected on one end to an anchor point and ran along the roof where it had a rope grab holding the other end of the line and the connector end of the rope grab was clipped into another anchor point. Was installed as a means of fall protection for employees performing wood framing.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. Site- Employees were exposed to fall hazards while using an 8 foot fiberglass step ladder to get between the third and fourth levels of the building which did not extend above the landing surface.
