(b)  The metal ladder frame was bent from the ground floor up to the ladder protection cage, the metal ladder frame was between 6 - 8 inches from center, thus no longer straight up and down.

(b) Employees engaged in walking on a catwalk platform located at 1300 Main Street, Atchison, Kansas between bin #5 and bin #6 were exposed to fall hazards between 89 - 94 feet from the catwalk platform to ground level due to no fall protection system being installed on the south side of the catwalk platform.  (c) Employees engaged in walking on a catwalk platform located at 1300 Main Street, Atchison, Kansas between bin #6 and bin #7 were exposed to fall hazards between 85 - 90 feet from the catwalk platform to ground level due to no fall protection system being installed on the south side of the catwalk platform.  (d) Employees engaged in walking on a catwalk platform located at 1300 Main Street, Atchison, Kansas between bin #7 and bin #8 were exposed to fall hazards between 82 - 87 feet from the catwalk platform to ground level due to no fall protection system being installed on the west side of the catwalk platform.  (e) Employees engaged in walking on a catwalk platform located at 1300 Main Street, Atchison, Kansas between the whole grain tank and millhouse leg were exposed to fall hazards of approximately 50 feet from the catwalk platform to ground level due to no fall protection system being installed on the west side of the catwalk platform.  (f) Employees engaged in accessing a catwalk platform from the whole grain tank located at 1300 Main Street, Atchison, Kansas were exposed to fall hazards of approximately 50 feet from the catwalk platform to ground level due to no mid-rail being installed on the north side of the whole grain tank.  (g) Employees engaged in accessing the catwalk platform from the whole grain tank located at 1300 Main Street, Atchison, Kansas were exposed to fall hazards of approximately 50 feet from the catwalk platform to ground level due to a 23 inch opening between the whole grain tank and the catwalk platform that was not guarded by a fall protection system.
