At the south side and south-west side of the building, a four frames high and two frames wide scaffold was bridged, using 2x10 planks, with a scaffold three frames high and three frames wide. The planking was bridged at 11 feet and was not placed in a manner that the overlap section was on a support.  Note: Non-mandatory Appendix A to this subpart contains examples of criteria that will enable an employer to comply with paragraph (a) of 29 CFR 1926.451.

At the south side of the building, employees were exposed to a 22 foot fall hazard, in that, a tubular welded frame scaffold three frames high and three frames wide with two bracket scaffold platforms attached had the scaffold legs at the east side rested on stacked loose boards. The scaffold was built inside the newly constructed pool which had an uneven bottom, on or about 02/13/2018.  b. At the west side of the building, employees were exposed to a 26 foot fall hazard, in that, a tubular welded frame scaffold four frames high and two frames wide had the outside scaffold leg at the south side rested on a loose board and a brick, on or about 02/13/2018.
