29 CFR 1926.652(a)(1)(ii): 29 CFR 1926.652(a)(1)(ii): Each employee in an excavation was not protected from cave-ins by an adequate protective system when the excavation was 5 feet (1.52 m) or more in depth. On or about 2/14/18 at the job site located at US Highway 71, Pineville, LA, the employer (Foreman) allowed an employee to enter and work in an unprotected trench preparing to make a bore (5' deep by 55" wide by 16' long) without having any cave-in protection; thus exposing employees to cave-in hazards.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. On or about 2/14/18 at the job site located at US Highway 71, Pineville, LA, the employer (Foreman) allowed an employee to enter and work in an unprotected trench preparing to make a bore (5' deep by 55" wide by 16' long) without having a ladder, ramp or other safe means of egress; thus exposing employees to a delay in exiting the trench.
