29 CFR 1910.22(a)(2):  Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:                                                                                                                                                                                                                                                                                                                                                                                           On or about January 18, 2018, working surfaces were not maintained in a clean and, so far as possible, a dry condition:    a)  Employees operating the Erco Machine Sheet metal/skid forming machine perform work on metal sections of the machine that are covered in oil based chemicals, exposing employees to slip and fall hazards.    b)  Employees preform work on at the Sheridan Skin Stretch Press where the walking surface is covered in oil based chemicals exposing employees to slip and fall hazards.

29 CFR 1910.22(a)(3):  Walking-working surfaces are not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.    On or about January 18, 2018, on the Sheridan Skin Stretch Press , employees were allowed to access and work from elevated platforms that had sections of the grating separating from the frame and protruding up.  Employees were exposed to fall and caught-by hazards.

29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems:  On or about January 18, 2018, at the facility, employees are required to work from and walk across metal sections of the machine that resemble metal beams and are sliding tracks (jaw slides) on the Erco machine. Employees are exposed to fall hazards between the jaw slides.
