29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      a) On or about February 15, 2018, the employer failed to ensure that the rotating disc on the Boyer Schultz H612 was adequately guarded to protect the operator. Employees were exposed to hazards from rotating parts and point of operation while operating the machine without a guard.

The employer uses scotch bright polishing wheels in the bench grinders and do not require additional guarding.    b) On February 15, 2018, the employer did not assure the end spindle nut on the bench grinders located near machine NH4000 DCG (#39) were properly guarded  The employer uses scotch bright polishing wheels in the bench grinders and do not require additional guarding.

29 CFR 1910.37(a)(3): Exit routes were not free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route.   a.) On or about February 15, 2018 the employer did not ensure that all exit routes were unobstructed.  The rear exit on the right side of the building behind CNC machine DM #1 was obstructed.  There was a lawn mower and several scrap drums/containers obstructing the exit route.
