29 CFR 1926.100(a): At this job site, employees exposed to possible head injury from falling or flying objects or from impact with hand tools, equipment and building materials during roofing operations were not wearing hard hats.   Employees were performing a residential roofing job located at 3127 Munson in Topeka, Kansas, the ground man was exposed to struck-by hazards while working below the overhead activities in that head protection was not provided and used.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  Employees performing a residential roofing job at 3127 Munson in Topeka, Kansas were exposed to fall hazards of 9 feet at the lowest eave to more than 20 feet.  The employees were working on the 8 x12 pitch roof with no fall protection in place.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Employees were performing a residential roofing job located at 3127 Munson in Topeka, Kansas; employees using the portable extension ladder to access the roof were exposed to fall hazards of approximately 9 1/2 feet from the roof to the surface below. The side rails of an extension ladder on the north-west corner of the structure extended approximately 6 inches above the landing surface of the roof.
