29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  (a) 4905 Clerkenwell Street Lot 1-Q, Montgomery, Alabama: On February 15, 2018 and times prior thereto the employer did not provide his employees with eye and face protection equipment when operations presented potential eye or face injury from physical agents. Employees were installing shingles on a roof using pneumatic nail guns.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) 4905 Clerkenwell Street Lot 1-Q, Montgomery, Alabama: Steep Slope Roof Surface: On February 15, 2018 and times prior thereto the employer failed to protect employees from falls of 15 feet to the ground below while installing roofing felt and shingles on a new roof. Employees had body harnesses and had installed lifelines but they were not tied off. Employees held lifelines when they were moving up and down the roof.
