29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 02/16/18 in the Production area; employees performed service and maintenance, such as but not limited to, changing the slitter knives, repositioning the slitter knives on the slitter, and were exposed to an amputation hazard when the employer did not require the use of a hazardous energy control procedure to control the hazardous energy prior to conducting service and maintenance work on the equipment. b) On or about 02/16/18 in the Production area; employees performed service and maintenance, such as but not limited to, changing the slitter knives on the eccentric slotter, and were exposed to an amputation hazard when the employer did not require the use of a hazardous energy control procedure to control hazardous energy prior to conducting service and maintenance work on the equipment. c) On or about 02/16/18 in the Production area; employees performed service and maintenance, such as but not limited to, changing the slitter knives on the partition slotter, and were exposed to an amputation hazard when the employer did not require the use of a hazardous energy procedure to control hazardous energy prior to conducting service and maintenance work on the equipment. d) On or about 02/16/18 in the Production area; employees performed service and maintenance, such as but not limited to, making adjustments to the rollers and guides, on the stitcher gluer and taper, and were exposed to an amputation hazard when the employer did not require the use of a hazardous energy control procedure prior to conducting service and maintenance work on the equipment.
