29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person: (a) 3 Woodlake Drive Shrewsbury, Pennsylvania - Employees worked on a residential roof without a fall protection system, employees were exposed to approximately a 20 foot fall to a lower level and falling through sky lights on the back roof. The employer did not have a competent person at the job site to inspected the job site and with the authority to correct hazards, on or about February 13, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The employer did not provide the employee with head protection, on or about February 13, 2018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Affordable Roofing was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 944782, citation number 1, item number 1 and was affirmed as a final order on July 28, 2014, with respect to a workplace located at 54 Piston Court, Stewartstown, PA 17363. No abatement certification or documentation required.
