An opening in a stair rail system exceeded 19 inches (48 cm) at its least dimension; a) Holtville Gin Co-Op; On or about February 14, 2018 and at times prior thereto the employer exposed employees to fall hazards in that employees were required to walk up and down fixed stairs that had a stair rail system with an opening that exceeded 19 inches at its least dimension in that there was no mid-rail.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Holtville Gin Co-op; On or about February 14, 2018 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were required to work on equipment to include but not limited to the cotton gin which had multiple energy sources such as electric and air without those employees having been provided with written machine specific lock out procedure(s) to utilize to ensure that those energy sources were controlled while the employees were performing maintenance or servicing on the machine(s).

The employer failed to report the work-related amputation to OSHA within twenty-four (24) hours of December 18, 2017 when the amputation occurred.
