29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: OR IN THE ALTERNATIVE 29 CFR 1910.212(a)(3)(ii): The point of operation was not guarded, designed, or constructed as to prevent the operator from having any part of his or her body parts in the danger zone during the operation: a) On the Main Line, on or about and at times preceding to February 12, 2018, an employee had a finger amputated when the drill and mash machine was nonfunctioning and the employee reached into the operation head and attempted to realign the head.
